PROJECT OBJECTIVES
The Prime Minister’s Challenge Fund has supported the establishment of ‘Best Care, Anywhere’, integrating primary care in Coventry. This aims to provide an integrated solution, improving primary care access and ensuring continuity of care through integrated pathways & interconnecting technology by setting up three high impact primary care schemes: (1) Extended hours hub, (2) Primary care frailty team, and (3) Primary care team in Emergency Department treating ambulatory cases.
The aim of this project is to use a mixed method approach incorporating patient and public engagement to comprehensively evaluate across the three intervention sites.
